Create and Submit Accident and Claim Forms, Documents, and Photos
From the Dashboard, scroll down until the Safety tile is visible, and tap the tile.
Tap the Report Accident tile, then tap Next, at the top right-hand corner of the screen.
Enter the information into the fields.
In order to proceed taking accident photos, the user must call 813-386-2319 to obtain a Reference Number.
Once complete, enter the number, then tap Next.
Tap the blue Add Existing button, to use photos and documents already stored on the mobile device.
Tap the green Take Photo button, to scan a new document or take a new photo.
Tap Next once complete.
From the Send Confirmation screen, tap Send All to submit forms, photos, and document images.
The user can add more photos or document image pages before submitting.
