From the Dashboard, tap the Safety tile.
This may be located in a different place, depending on your fleet’s configuration.
On the Safety Main Menu, the user has the options to report an accident, report OS&D, report any tire damage, and access the safety videos.
From the Safety Main Menu, tap Report Accident.
The Instructions modal appears.
Tap Call Hotline to obtain your Reference Number for the accident event.
Once you have completed the call, tap Next.
On the next screen, fill out the details for the accident, then tap Next.
If you do not include a Reference Number, the Error modal will appear.
From the Safety Main Menu, tap Report OS&D.
On the Instructions screen, tap Call OS&D, to report the incident.
Tap Next once complete.
Scan documents and take any photos if necessary.

After scanning any forms and taking photos, tap Send All to receive the Confirmation Number for your documents.
Report Tire Damage
From the Safety Main Menu, tap Report Tire Damage.
Select Dual Tire or Wide Base for the Truck.
Tap on any of the tires where the damage has occurred.
On the new screen, you have the option to name the tire.
The Driver ID is already pre-populated.
Tap Next to take photos of the tires.

After taking photos of the tire(s), tap Send All to retrieve your confirmation number.
From the Safety Main Menu, tap Safety Videos.
The next screen displays the YouTube channel for TRANSFLO®, with educational safety videos for Drivers.
